Our Patient Portal permits you to request prescription refills, view and download documents related to your medical care, and see all future scheduled appointments.

Our system will authenticate you with the following information: last four digits of your social security number, your primary telephone number, your DOB, and your email address. 

Once you have logged on to the portal for the first time, we would greatly appreciate if you would send us a “Hello” in the message section. This will notify us that you have successfully accessed the portal.

Please do not send any messages or questions about your medical care through the portal. These will not be addressed or answered via the portal.
